Health information you share as a patient is treated as protected health information and handled in accordance with applicable federal and Illinois privacy laws. Scheduling, visits and medical records are managed through our secure electronic health record and patient portal. A separate Notice of Privacy Practices is provided to patients.

Booking, telehealth visits and records are handled by our electronic health record provider, and links to outside sites such as Instagram are governed by their own privacy policies.

We use reasonable administrative, technical and physical safeguards to protect your information and keep it only as long as needed for care, legal and record-keeping requirements. No method of transmission over the internet is completely secure.

You may ask to access, correct or delete the non-medical information you have shared with us by emailing bo2gochi@gmail.com. Requests involving medical records follow applicable health-record laws.
