Sexual and vaginal health in perimenopause and menopause

Dryness, burning, painful sex, recurrent urinary symptoms and lower desire are common and treatable. They are a medical issue, not something to be embarrassed about.

Genitourinary syndrome of menopause (GSM)

Lower estrogen thins and dries the tissues of the vulva, vagina and bladder. Unlike hot flashes, these symptoms usually do not improve on their own and often progress. GSM can cause dryness, irritation, pain with intercourse, urgency and recurrent urinary tract infections.

Treatment options

Depending on your symptoms and history, options may include:

  • Vaginal moisturizers and lubricants
  • Low-dose vaginal estrogen, which acts locally with minimal absorption into the bloodstream
  • Vaginal DHEA or other prescription options
  • Referral to pelvic floor physical therapy when helpful
  • Evaluation of low desire, including whether testosterone may be appropriate
